ParQ forms need to be updated each September or whenever you have a change in your health. You can complete this online below or download & print the form below to give to me at the start of the new term. If you are unable to do either please read the form and email me with any relevant details and I will bring a form to be completed before the first class.

Please click through the order as if you are going to pay and then select Bank Payment or Cash options and make your payment as usual.
